• Print the form, fill it out by hand; then fax it to us using the number on the form
• Enter your information onscreen, then print and fax it to us
• Enter your information onscreen, then save the PDF and e-mail it to us

You are also welcome to use the form as a guideline for gathering your information, and call us to place your submission by phone.  This may actually save time as the mediator can help you with the discovery process covered on the second page.
